The mission of the Tri-County Diabetes Alliance is to raise awareness and improve the health of people with diabetes and those at risk for developing diabetes by creating and sustaining an effort to inform, educate, and assist individuals to improve their health.
Who is the Tri-County Diabetes Alliance?
The Tri-County (Somerset, Wicomico, and Worcester Counties)
community of the Eastern Shore of Maryland have united together to
help lower the number of Tri-County residents with diabetes. At an
alarming rate of 14.3 % of the Tri-County citizens being
diagnosed with diabetes, almost twice the national rate, health
professionals recognized the need in this area for a targeted
diabetes awareness, prevention , and educational program. Thus, a
unification of health professionals to address the need to reduce
the alarming diabetes rate within the community was born.
This unification became known as the
Tri-County Diabetes Alliance and has been in existence since 2004.
The Tri-County Diabetes Alliance is made up of both local
health care professionals and community volunteers! The
local organizations include: Atlantic General Hospital,
McCready Hospital,
Peninsula Regional Medical
Center, the Somerset,
Wicomico, and Worcester County Health Departments, The Eastern Shore Division of the
American Diabetes Association,
Salisbury Urban Ministries, University of Maryland Eastern Shore,TLC (Three Lower Counties), and
volunteer citizens from the Eastern Shore of Maryland.
