Mission Statement

The mission of the Tri-County Diabetes Alliance is to raise awareness and improve the health of people with diabetes and those at risk for developing diabetes by creating and sustaining an effort to inform, educate, and assist individuals to improve their health.

Goals & Objectives

The Tri-County Diabetes Alliance will follow the following Healthy People 2010 guidelines:

Healthy People 2010:

HP 2010 – 5-1 Increase the proportion of persons with diabetes who receive formal diabetes education.

HP 2010 – 5-2 Prevent diabetes.

HP 2010 – 5-4 Increase proportion of adults with diabetes whose condition has been diagnosed.

I. The Tri-County Diabetes Alliance will identify individuals at risk of developing Diabetes in the Tri-County region of Wicomico, Worcester and Somerset Counties in Maryland.

Objective 1: By FY2010, the Tri-County Diabetes Alliance will increase the number of individuals identified or considered at risk of developing diabetes by 30% or 600 individuals.

Activities:
1. The Tri-County Diabetes Alliance and its partner members will administer the American Diabetes Association risk assessment paper screening test to individuals at a variety of settings throughout the tri-county region. The alliance will reach a total of 300 individuals by the end of FY09.
2. The Tri-County Diabetes Alliance and its partners will establish and form partnerships with at least three new professional and private organizations or individuals by the end of FY10 that will assist in identification of at-risk individuals.
3. The Tri-County Diabetes Alliance and its partners will focus efforts in five areas; business, school, faith, professional, and general community, that will assist in identification of at-risk individuals.
a. Business - The Alliance will send 100 letters to workplaces in the tri-county area highlighting the impact of diabetes on work.
b. School – The Alliance will host a pilot program to outreach to an elementary school for a lunch and learn.
c. Faith – The Alliance will work with local Health Ministries to find at-risk individuals.

e. General Community – see Goal II, Objective I

II. The Tri-County Diabetes Alliance will educate the general community, the underserved and at risk individuals in the Tri-County region of Wicomico, Worcester, and Somerset Counties, about pre-diabetes, diabetes, and the prevention of diabetes.

Objective 1: Through primary prevention efforts, the Tri-County Diabetes
Alliance by FY 09 will educate over 1,000 individuals.

Activities:
1. The Tri-County Diabetes Alliance will continue to publish and distribute resource guides at various community outreach forums.
2. The Tri-County Diabetes Alliance will participate in approximately 3 health fairs in the Tri-County area (ie Road Shows)
3. The Tri-County Diabetes Alliance will continue to fund and update the website for general public education on Diabetes.
4. The Tri-County Diabetes Alliance will design, establish, and publish a Social Marketing Campaign called Closing the Gap on Diabetes to create a change in social norm through nutrition, exercise and appropriate testing strategies.
5. The Tri-County Diabetes Alliance will actively recruit volunteers, lay people, and those who have an active interest in Diabetes to assist with identification and education.

Objective 2: By FY09, the Tri-County Diabetes Alliance will educate 30 professionals on the Alliance goals and Diabetes information.

Activities:
1. The Tri-County Diabetes Alliance will seek out avenues in which to educate professionals. (ie, Poster Sessions, Speaking Engagements whether state or national, and policy committees, etc.)
2. The Tri-County Diabetes Alliance will participate in one formal presentation for professionals called the Tri-County Summit.
3. The Tri-County Diabetes Alliance will seek to partner with national organizations to provide professional education (CME, CEU’s, etc.) training on the latest in Diabetes prevention, care, and management of the chronic disease.
a. Partner can consist of Pharmaceutical companies, Universities, and State entities.

Objective 3: By FY09, the Tri-County Diabetes Alliance will educate 10 policy-makers in regards to the effects, disparities, and cost of Diabetes in the area.

Activities:
1. The Tri-County Diabetes Alliance will host a breakfast forum for all state, county, and local legislatures in regards to Diabetes prevalence, incidence, and cost.
2. The Tri-County Diabetes Alliance will recruit school-board members, nutrition policy committee members from schools, and/or school health nurses to join the Alliance to discuss change.

III. As a long-range goal, The Tri-County Diabetes Alliance will increase the proportion of individuals with Diabetes whose condition has been diagnosed.

Objective 1: By 2012, with appropriate funding sources, the Tri-County Diabetes Alliance will design, establish, and maintain 3 Diabetes clinics; one in each of the three lower shore county health department’s or hospital’s.

Objective 2: By 2012, Paraprofessionals by county will be trained to assist with identification and education in order to sustain the diabetes prevention effort in the Tri-County communities.

This web resource focuses on  preventing, and guiding the local community of the three lower counties of the Eastern Shore of Maryland on their own individual risk of developing diabetes.  The Information contained on this web site is not intended as a substitute for medical advice or treatment. Specific Web Site Goals

 The purpose of this website is  as follows:

  • Educate citizens on why it is important to know if you have diabetes.

  • Guide citizens to community resources to learn about your risk of developing diabetes.

  • Guide citizens to community services to help them manage their diabetes if diagnosed.

This web site was developed by a group of health professionals from Somerset, Wicomico, and Worcester Counties in Maryland in coordination with the local chapter of the American Diabetes Association.

The Tri-County Diabetes Alliance sponsors and conducts several health education and outreach programs addressing prevention of pre-diabetes and diabetes. This will include presentations and free health screenings in a variety of settings including schools, worksites, faith based communities, and other community centers.
