Mission Statement

The mission of the Tri-County Diabetes Alliance is to raise awareness and improve the health of people with diabetes and those at risk for developing diabetes by creating and sustaining an effort to inform, educate, and assist individuals to improve their health.
 

Below there are several diabetes education programs, and support groups in the Tri-County area that include trained diabetes educators to teach you about how to live with and successfully manage your condition.  You can also visit our Internet Resources page by clicking here to get additional on-line support.

Home Bound Patients with Diabetes:

For patients with diabetes who are house-bound, Medicare and private insurance (in most cases) allow a home health nurse to do diabetes management teaching in the home.
